My (40M) wife (35F) is ill, needs constant care. Can't do it by myself anymore.

We've seen doctors, and specialists. Had test after test. No answers for several years. Nobody can figure out what's wrong, and I'm perpetually exhausted. I am trying to manage the house, and work, and take care of her. It's gotten to the point now where we are constantly annoyed with each other; me due to her constant needs, and her due to me asking her to wait while I finish whatever I'm doing. I know the constant stress isn't helping either of us. I can't even take some time away without feeling guilty about it. Help!
